Before filling out the form, make sure you have clear digital copies (JPG, PNG, or PDF) of the following:

✅ Valid ID (Government-issued ID)

✅ Proof of Residency (Utility bill, Barangay Certificate, or Lease Agreement)

📎 Additional documents may be required depending on the type of request.

Click the “Request Document Now” button and complete the following sections:

Data Privacy Notice – Read and agree to proceed.

Resident Information Form – Provide your personal details and upload the necessary files.

Document Request Form – Select the document you need and review its specific requirements.

After submitting your form: 

You’ll receive a confirmation email acknowledging your request.

The barangay staff will review and verify your submitted documents.

Once your request is approved and ready, you’ll receive a text or email message containing your pickup schedule.

🕐 Processing is available on weekdays, from 8:00 AM to 5:00 PM.

1. Your Valid ID (original)

2. The confirmation message or text notification
